  • When you’re logged in to EyeV, click Refer to a Service from the Home Screen.
  • Then, click Make GOS18 Referral from the list of services.
  • You’ll then need to search for a patient on the NHS Spine; you’ll need the patient’s name, date of birth, gender and postcode.
  • Once the patient is confirmed in the search, you can then click This is the correct patient, and make the referral.
  • The referral form is comprised of the mandatory fields from the standard GOS 18 form, and additional non-mandatory fields (such as colour vision and Amsler chart), which are there to improve the quality of referrals.
  • You can set the priority of the referral from the How quickly does this patient need to be seen option? The referral priorities will be set by the region, but will generally be Routine, Two Week Wait, Urgent, Emergency.

  • Click View to view the referral, and access any triage actions.
  • A triage summary and any images can be downloaded from this screen.
  • Click Accept / Redirect, or Reject, depending on the most appropriate course of action for the referral.
  • Once you’ve completed the triage fields, click Submit at the bottom of the page. This will automatically perform any relevant triage actions, and inform the referring practice of the outcome.
